Why use a student information sheet, parent contact information sheet, or a student survey? These 7 documents help you communicate with parents and make your back-to-school transition easy. A good way to make lessons relevant to the students is by using this editable Student Information Sheet in English and Spanish and Student Interest Survey. Students get hooked into the lesson when they see themselves or topics of interest to them. At the beginning of the year (or any time), students fill in the sheet with their personal information, phone numbers, parents’ names, numbers, and classes so the information is instantly accessible when the teacher needs it.
There are four levels of student information sheets English and Spanish: Spanish 1, 2, 3, and 4. For Spanish 1 and 2, the information sheet provides the Spanish and the English. For Spanish 3 and 4, only Spanish is written.
The student survey can be used for all levels.

HOW I USE THIS INFORMATION TO IMPROVE MY LESSONS:
STUDENT INFORMATION SHEET ENGLISH AND SPANISH
- BACK TO SCHOOL OR ANY TIME: At the beginning of the school year, semester, or class, students write in their information on the sheet. I keep the parent contact information sheets in a binder. That way, whenever I need to call a parent, I have the information quickly at hand.
- If a student is struggling, I can gather information about the student from the previous teacher. Or, get an idea of what the problem might be, depending on who the teacher is. (I know—politically incorrect. But if I know a teacher uses mostly English in the classroom, for example, I will provide more listening opportunities to the student or suggest a tutor.)
- If I need to contact the student during my prep, I know which class he or she is in during that period.
- If I want to send home a letter or an email praising the student, I have easy access to the information.
- If I want to email a student for any reason, I don’t have call the counselor or search online to get that information.
STUDENT SURVEY / STUDENT INTEREST INVENTORY
- The editable interest inventory helps me spice up my lesson plans with topics that interest students. Every day, for a minute or two, I look in the binder to see what interests or cultural icons are popular with the students. I insert those into the lesson and grab students’ attention.
